Welcome to the Virtual Medical Director/Designated Physician Case Initiation. If one of your employees has suffered a workplace injury, is absent or has submitted an accommodation request or is stuck turning in circles with a medical issue, getting started is easy:

1. To initiate the Virtual Medical Director or Designated Physician Service, download, complete and sign the Case Initiation forms below.
2. Once all the documents have been completed and signed, they should be emailed to Advocate@medextra.com
